  • Definitions – hazards, accidents, near miss
  • Accident causation models – domino theory and multi causality
  • Immediate and underlying protocol – what should be investigated, and by who
  • Accident investigation procedure:
  • Immediate action following accident
  • Level of investigation and person involved
  • Gathering detailed information including photographs and witness statements
  • Analysing the information
  • Identifying suitable risk control measures
  • Action plan and implementation
  • Practice exercise on accident investigation
  • End of course assessment


Delegates will be awarded a certificate of attendance on successful completion of assessment.


The course is normally held on client’s premises, and delivered specifically for their employees. If required, the course can be held on Phoenix Health & Safety premises at an additional cost. Travel and accommodation costs may be charged additionally if the location is outside England or Wales.
